HEALTH CARE PROVIDERS & HELP ME GROW

Help Me Grow is a parent information line designed to connect families to information about child development and community resources. As a healthcare provider, you work with so many families that need to be connected to various resources, not just medical resources. Help Me Grow supports you by making referrals and then following up with the families to make sure that they received help appropriate to their concerns and circumstances. We also provide parents with a developmental screening tool that helps them understand typical child development. To strengthen the relationship between you and your patient, we will always fax providers (with family’s permission) a summary of the completed developmental questionnaire, as well as any resources or services we were able to connect the family referred.

How do I refer a family to Help Me Grow Alabama?

Discuss developmental screening results (ASQ-3, ASQ:SE-2, M-CHAT) and/or any concerns identified with the family
Give the family HMG information and explain to the family that a HMG care coordinator will be contacting them to answer questions, discuss concerns, and connect them with information or local community resources
Email or fax the following supporting documentation to regional care coordinators
- HMG referral form (completed with parent signature)
- Developmental screenings (full questionnaire-not summary sheet)

Save you time! We stay up to date with community resources so you spend your time working with patients.
Follow-up with families to make sure services are utilized.
Benefit from additional care coordination. Help your patients navigate resources beyond the walls of your practice, and receive updates on their progress.
We can provide ASQ surveillance by sending patients who need to be monitored between well-child checks follow up ASQ’s and, with the permission of the client, we’ll send you the results.
Support your Maintenance of Certification (MOC).
Help your families become educated, responsible and self-reliant.
